Using a true LEADER object ?

Not that I'm aware of.

You could probably write a LISP routine to draw the border, but it wouldn't be a true multileader.

Using a dynamic block could be done. You make the leader part a visibility state. 

 

Civ3D does this for some label styles but at times it just not right.

 

I did a circle, text inside then leader went from centre to point and was trimmed at circle edge so line was radial, could be done for the image shown. Like Cyberangel a lisp.
