What's so special about the annotation that appears as an image in your post? My guess is it is either a block or a dynamic block.

I would assume it's an annotative block. They're not hard to make really. There are tutorials online all over the place for annotative blocks.

Assuming you are asking about GD&T, if not ignore. You can use TOLERANCE command for the M and for the other frames, the line and rectangle will be made by you. Draw it and create a block.

 

Multileader would be a good way to go, pretty easy to get set up.
