  1. This could possibly be an annotation issue. When it's annotative, an object has a specific scale, and it will not be visible unless it's in a space set to the same scale. When you select the object, you can see any versions that have other scales. Check the Properties for M3. See if the Annotative property is On. Check the annotation scale for model space (you may have to enable the combo box). Do some research on annotative text. If this doesn't put you on the right track, we'll take another shot at it.   3. You have posted that you have LT so it is possible to write scripts and run them in LT, you would use say Excel to create the script. You can edit a block using -attedit. I don't have LT so hopefully a script like this will work. This is for one attribute. In my title block the layout names are D01D02 D88 etc and the title block is always at 0,0. The X,Y value is a point on top of the attribute to be changed. Note the blank lines they are required.
